Hello! New York State has launched the Social Care Network (SCN) program to provide Medicaid members with more comprehensive support beyond traditional Medicaid medical services. This initiative helps individuals access essential resources such as food, housing, and transportation to improve overall health and quality of life.
Asian Community Care Organization (ACCO) helps Medicaid members complete screenings and connects them to services such as meal delivery, pantry stocking, nutrition counseling, rental assistance, and home modifications.
If you are a Medicaid member living in Manhattan, Queens, Brooklyn, Staten Island, or Long Island and are interested in receiving free services from ACCO, please provide your name and contact information below.
